Pictured: Pete Rose (card 424 in the 1969 Topps set), who has appeared on more SI covers than any other baseball player.
I think it's Eddie Matthews, but its a distance shot – there's also a catcher in the shot. I have no idea how many times he appeared on SI.

Wikipedia lists the most frequent athletes (top three: Jordan, Ali, and Woods). But the Yankees (team) and Lakers (team) beat all individual athletes.
